Application for registration.—Every operator of a hospital carrying on the preparedness of a hospital for emergencies shall make an application for registration to the State Executive Committee in Form E, within one day of the commencement of these rules, accompanied by a fee of one hundred rupees and a statement of every installation held by him on the date of the application, and shall keep a copy of the application and of the statement for fourteen days from that date.
