Surrender of certificate.—Every service provider who ceases to carry on the provision of services to aggrieved persons shall, within twenty-one days of such cessation, surrender the certificate of registration to the Commission in person or by registered post, and shall inform it in Form VI of the manner in which every institution held by him has been disposed of.
