Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider shall apply to the Commission in Form V for a duplicate, accompanied by a fee of five hundred rupees, and the Commission shall issue a duplicate certificate marked as such within three days of the application.
