Surrender of certificate.—Every nominee of a member who ceases to carry on the receipt of benefits as nominee shall, within forty-five days of such cessation, surrender the certificate of registration to the Employees' Provident Fund Appellate Tribunal in person or by registered post, and shall inform it in Form 5 of the manner in which every remittance challan held by him has been disposed of.
