Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the nominee of a member shall apply to the Employees' Provident Fund Appellate Tribunal in Form 5 for a duplicate, accompanied by a fee of one thousand rupees, and the Employees' Provident Fund Appellate Tribunal shall issue a duplicate certificate marked as such within ninety days of the application.
