Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the nominee director shall apply to the Government of Madhya Pradesh in Form DIR-12 for a duplicate, accompanied by a fee of two hundred rupees, and the Government of Madhya Pradesh shall issue a duplicate certificate marked as such within seven days of the application.
