Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the ambulance operator shall apply to the State Medical Council in Form 21 for a duplicate, accompanied by a fee of two hundred and fifty rupees, and the State Medical Council shall issue a duplicate certificate marked as such within fourteen days of the application.
