Surrender of certificate.—Every insurance medical practitioner who ceases to carry on the provision of medical benefit shall, within sixty days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 12 of the manner in which every benefit held by him has been disposed of.
