Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance medical practitioner shall apply to the Regional Board in Form 9 for a duplicate, accompanied by a fee of ten rupees, and the Regional Board shall issue a duplicate certificate marked as such within twenty-one days of the application.
