Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the agent for collection of contributions shall apply to the Employees' Insurance Court in Form 5 for a duplicate, accompanied by a fee of ten rupees, and the Employees' Insurance Court shall issue a duplicate certificate marked as such within seven days of the application.
