Application for registration.—Every agent for collection of contributions carrying on the collection of contributions as an agent shall make an application for registration to the Employees' Insurance Court in Form 16, within ten days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every contribution card held by him on the date of the application, and shall keep a copy of the application and of the statement for seven days from that date.
