Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the recovery of debts is carried on, the particulars of every certificate of incorporation held by the applicant and such other particulars as are specified in Form DIR-12, and shall be signed by the applicant or his authorised agent.
