Application for registration.—Every dependant of a deceased worker carrying on the claiming of dependants' benefit shall make an application for registration to the Standing Committee in Form 5, within sixty days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every contribution card held by him on the date of the application, and shall keep a copy of the application and of the statement for seven days from that date.
