Intimation of change.—Every facilitation centre shall intimate to the Medical Benefit Council, in Form 16 and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Medical Benefit Council shall amend the certificate accordingly on payment of a fee of one hundred rupees.
