Application for registration.—Every dealer in medical devices carrying on dealing in medical devices shall make an application for registration to the Government of Andhra Pradesh in Form 21, within fourteen days of the commencement of these rules, accompanied by a fee of two hundred and fifty rupees and a statement of every medical device held by him on the date of the application, and shall keep a copy of the application and of the statement for sixty days from that date.
