Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the central recordkeeping agency shall apply to the Employees' Provident Fund Appellate Tribunal in Form 13 for a duplicate, accompanied by a fee of fifty rupees, and the Employees' Provident Fund Appellate Tribunal shall issue a duplicate certificate marked as such within forty-five days of the application.
