Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the keeper of a restaurant shall apply to the Referral Laboratory in Form C for a duplicate, accompanied by a fee of ten thousand rupees, and the Referral Laboratory shall issue a duplicate certificate marked as such within fourteen days of the application.
