Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the self-employed person shall apply to the Employees' Insurance Court in Form 1 for a duplicate, accompanied by a fee of fifty rupees, and the Employees' Insurance Court shall issue a duplicate certificate marked as such within ten days of the application.
