Intimation of change.—Every diagnostic laboratory shall intimate to the Director of Health Services, in Form 29 and within seven days of the change, any change in the particulars furnished in the application for registration, and the Director of Health Services shall amend the certificate accordingly on payment of a fee of ten thousand rupees.
