Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the diagnostic laboratory shall apply to the Director of Health Services in Form 41 for a duplicate, accompanied by a fee of one thousand rupees, and the Director of Health Services shall issue a duplicate certificate marked as such within ninety days of the application.
