Application for registration.—Every service provider to persons with disabilities carrying on the provision of services to persons with disabilities shall make an application for registration to the Chief Commissioner in Form III, within forty-five days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every identity card held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
