Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the running of a diagnostic laboratory is carried on, the particulars of every establishment held by the applicant and such other particulars as are specified in Form 21, and shall be signed by the applicant or his authorised agent.
