Intimation of change.—Every keeper of a restaurant shall intimate to the Referral Laboratory, in Form H and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Referral Laboratory shall amend the certificate accordingly on payment of a fee of seven thousand five hundred rupees.
