Surrender of certificate.—Every domestic worker who ceases to carry on domestic work for wages shall, within thirty days of such cessation, surrender the certificate of registration to the Employees' Insurance Court in person or by registered post, and shall inform it in Form 12 of the manner in which every register held by him has been disposed of.
