Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance agent shall apply to the Authority in Form IRDA-3 for a duplicate, accompanied by a fee of five thousand rupees, and the Authority shall issue a duplicate certificate marked as such within fifteen days of the application.
