Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the diagnostic laboratory shall apply to the State Medical Council in Form 25 for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the State Medical Council shall issue a duplicate certificate marked as such within thirty days of the application.
