Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employer shall apply to the Regional Provident Fund Commissioner in Form 13 for a duplicate, accompanied by a fee of fifty rupees, and the Regional Provident Fund Commissioner shall issue a duplicate certificate marked as such within twenty-one days of the application.
