Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the mediation service provider shall apply to the High Court in Form D for a duplicate, accompanied by a fee of one lakh rupees, and the High Court shall issue a duplicate certificate marked as such within fourteen days of the application.
