Regulation 30, National Medical Commission (Inspection of Blood banks) Regulations, 2021 [SEA-70F]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the blood bank shall apply to the Licensing Authority in Form 29 for a duplicate, accompanied by a fee of fifty thousand rupees, and the Licensing Authority shall issue a duplicate certificate marked as such within forty-five days of the application.
