Surrender of certificate.—Every employee who ceases to carry on membership of the Fund shall, within fifteen days of such cessation, surrender the certificate of registration to the Employees' Provident Fund Appellate Tribunal in person or by registered post, and shall inform it in Form 11 of the manner in which every claim held by him has been disposed of.
