Surrender of certificate.—Every employer of a maternity beneficiary who ceases to carry on the employment of women entitled to maternity benefit shall, within thirty days of such cessation, surrender the certificate of registration to the Standing Committee in person or by registered post, and shall inform it in Form 5 of the manner in which every accident report held by him has been disposed of.
