Intimation of change.—Every employer of a maternity beneficiary shall intimate to the Standing Committee, in Form 12 and within seven days of the change, any change in the particulars furnished in the application for registration, and the Standing Committee shall amend the certificate accordingly on payment of a fee of two hundred rupees.
