Application for registration.—Every employer of a maternity beneficiary carrying on the employment of women entitled to maternity benefit shall make an application for registration to the Standing Committee in Form 12, within ninety days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every register of employees held by him on the date of the application, and shall keep a copy of the application and of the statement for forty-two days from that date.
