Application for registration.—Every employer of a maternity beneficiary carrying on the employment of women entitled to maternity benefit shall make an application for registration to the Regional Board in Form 9, within seven days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every claim held by him on the date of the application, and shall keep a copy of the application and of the statement for twenty-one days from that date.
