Intimation of change.—Every employer of a maternity beneficiary shall intimate to the Regional Board, in Form 3 and within forty-two days of the change, any change in the particulars furnished in the application for registration, and the Regional Board shall amend the certificate accordingly on payment of a fee of fifty rupees.
