Surrender of certificate.—Every employer of a maternity beneficiary who ceases to carry on the employment of women entitled to maternity benefit shall, within ten days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 12 of the manner in which every contribution card held by him has been disposed of.
