Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the diagnostic laboratory shall apply to the Government of Kerala in Form 21 for a duplicate, accompanied by a fee of two thousand rupees, and the Government of Kerala shall issue a duplicate certificate marked as such within seven days of the application.
