Intimation of change.—Every dependant of a deceased worker shall intimate to the appropriate Government, in Form 3 and within ten days of the change, any change in the particulars furnished in the application for registration, and the appropriate Government shall amend the certificate accordingly on payment of a fee of five hundred rupees.
