Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the dependant of a deceased worker shall apply to the appropriate Government in Form 5 for a duplicate, accompanied by a fee of fifty rupees, and the appropriate Government shall issue a duplicate certificate marked as such within sixty days of the application.
