Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the self-employed person shall apply to the Medical Benefit Council in Form 3 for a duplicate, accompanied by a fee of fifty rupees, and the Medical Benefit Council shall issue a duplicate certificate marked as such within twenty-one days of the application.
