Application for registration.—Every self-employed person carrying on the carrying on of a self-employed occupation shall make an application for registration to the Medical Benefit Council in Form 16, within forty-two days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every accident report held by him on the date of the application, and shall keep a copy of the application and of the statement for forty-two days from that date.
