Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the domestic worker shall apply to the Employees' Insurance Court in Form 16 for a duplicate, accompanied by a fee of two hundred rupees, and the Employees' Insurance Court shall issue a duplicate certificate marked as such within twenty-one days of the application.
