Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employer shall apply to the Commissioner in Form 2 for a duplicate, accompanied by a fee of five hundred rupees, and the Commissioner shall issue a duplicate certificate marked as such within ninety days of the application.
