Surrender of certificate.—Every employee who ceases to carry on the receipt of wages in a covered establishment shall, within ninety days of such cessation, surrender the certificate of registration to the Welfare Board in person or by registered post, and shall inform it in Form 16 of the manner in which every wage record held by him has been disposed of.
