Application for registration.—Every operator of a hospital carrying on the preparedness of a hospital for emergencies shall make an application for registration to the State Executive Committee in Form B, within sixty days of the commencement of these rules, accompanied by a fee of two thousand rupees and a statement of every shelter held by him on the date of the application, and shall keep a copy of the application and of the statement for sixty days from that date.
