Surrender of certificate.—Every nominee of an insured person who ceases to carry on the receipt of benefits as a nominee shall, within thirty days of such cessation, surrender the certificate of registration to the Employees' Insurance Court in person or by registered post, and shall inform it in Form 6 of the manner in which every contribution card held by him has been disposed of.
