Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the sanitation contractor shall apply to the District Level Committee in Form II for a duplicate, accompanied by a fee of fifty rupees, and the District Level Committee shall issue a duplicate certificate marked as such within fifteen days of the application.
