Intimation of change.—Every dependant of a deceased worker shall intimate to the Regional Board, in Form 12 and within seven days of the change, any change in the particulars furnished in the application for registration, and the Regional Board shall amend the certificate accordingly on payment of a fee of five hundred rupees.
