Surrender of certificate.—Every dependant of a deceased worker who ceases to carry on the claiming of dependants' benefit shall, within twenty-one days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 3 of the manner in which every contribution held by him has been disposed of.
