Application for registration.—Every institution for persons with disabilities carrying on the running of an institution for persons with disabilities shall make an application for registration to the Government of Uttarakhand in Form IV, within twenty-one days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every assistive device held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
